CCHC Seeks Community Benefits Grant Applications

Cape Cod Healthcare has released its Community Benefits FY14 Request for Proposal (RFP). This RFP is part of an annual competitive grant process focused on addressing unmet health needs of the residents of Barnstable County.
CCHC annually solicits Community Benefits grant applications from non-profit organizations. Applicants must demonstrate their value to the community in the way of health and human services benefits. CCHC will award up to $200,000 in this cycle, with grants ranging up to a maximum of $30,000. To qualify, proposals must meet at least one of the following six guidelines:
• Positively impact chronic and infectious disease management.
• Expand access to healthcare services for vulnerable populations.
• Support initiatives targeted at individuals and families facing mental health issues.
• Support substance abuse prevention and intervention strategies.
• Address senior health challenges.
• Address health and at-risk behaviors among youth and young adults.
In addition to this grant program, CCHC’s Community Benefits’ efforts include hospital-based programs, collaboration with community health centers, regional health and human service partnerships, and support of community health programs to meet its mission of enhancing the quality of and access to comprehensive healthcare services for all residents of Barnstable County.
