The Cape Cod Healthcare Dementia & Alzheimer’s Caregiver Support program offers a wide range of services for caregivers managing the many challenges of caring for a family member with dementia.

How we can help

Caregiver InfographicResearch has demonstrated that when caregivers are engaged in support and education they are better able to keep their loved one home anywhere from 6 months to 5 years longer than caregivers who do not have support.

The CCHC Dementia & Alzheimer’s Support Program places the caregiver at the center of our care model surrounding them with a wide range of programs and services. We are a care management program helping caregivers to understand and manage the many challenges that come with caring for a loved one with dementia.

Though Alzheimer’s disease is the most common type of dementia, there are as many as 50 other forms of dementia often creating confusion for caregivers. Regardless of the dementia diagnosis we are here to help you understand your role as a caregiver while providing you with support and guidance to manage the challenges that lie ahead.

Individual and Family Consultations

As part of the Cape Cod Healthcare family of services, our program is able to receive referrals and, in most cases, communicate directly with your primary care doctor and neurologist to provide continuity of care throughout the disease process. 

Any caregiver managing the care of someone with dementia can also call and refer themselves to the program. All services are free of charge.
