Falmouth Hospital Medical Center at Bramblebush offers an option for care when you need it from the experienced hospitalist team at Falmouth Hospital - from routine primary care and acute concerns to pre-operative clearance and follow up after you leave the hospital, and much more.

Located on the campus of Falmouth Hospital, this innovative practice is staffed by a provider team consisting of board-certified internal medicine physicians and advanced practice clinicians (APCs) who provide both inpatient care at Falmouth Hospital and outpatient care at the practice. Because this practice utilizes the same model of shared care as in the hospital setting, patients will be cared for by a team of providers to manage all medical care needs, as an alternative to a traditional model where a patient is assigned to one primary care provider.

The well-established team of hospitalists and APCs rotate coverage between the medical center and the hospital, offer an experienced skill set, and are versed in working together to provide fluid continuity of care, whether a patient requires primary care or other outpatient services, or inpatient services at Falmouth Hospital.

Our hospitalist team

Led by Vladimir Koren, MD, Director and Chief of the Department of Hospital Medicine at Falmouth Hospital, the expert team of hospitalist providers at Falmouth Hospital Medical Center at Bramblebush consists of board-certified internal medicine physicians and advanced practice clinicians (physician assistants and nurse practitioners) dedicated to the continuity of patient care.

A hospitalist is traditionally an extension of a patient’s primary care physician (PCP), consulting with one’s PCP on aspects of care should admission to the hospital be necessary. As a patient of Falmouth Hospital Medical Center at Bramblebush, your team provides fluid care – from routine, office-based primary care through inpatient hospital-based medicine if required. In either setting, this team will consult with specialists when needed, monitor test results and recommended screenings, discuss treatment options and quickly respond to changes in your health status. As you will be cared for by more than one hospitalist provider, our team will get to know you and vice versa, helping to further patient care continuity and communication.
