Helping Hands - Your Care Coordinator

Living with a chronic health condition can be challenging. That’s why our Helping Hands Program is dedicated to providing comprehensive support and guidance to every CCHC Employee Health Plan (EHP) Member.

Our experienced healthcare professionals offer Care Coordination, which is an active approach to optimize the health and wellness of individuals, who are suffering from one or more chronic conditions. Eligible Members will receive concierge support services from a Nurse Care Coordinator, who will partner with your Primary Care Physician (PCP) to develop a health and wellness plan tailored to your needs.

Our program is designed for individuals who may be at risk if their health is not proactively managed.

Eligible members will receive concierge support services from a Nurse Care Coordinator.

The Nurse Care Coordinators are all Registered Nurses located in our community. They have extensive experience and success, in working with individuals who strive to live their lives to the fullest, but just need a "Helping Hand". The Nurse Care Coordinators will advocate for Members throughout the continuum of care working with member’s care givers/families, physicians, and community resources to empower Members to reach their health and wellness goals.

Coordinated Care Program

Care coordination is the concept of healthcare professionals partnering with patients to ensure that patients health needs are being met and that the right provider is delivering the right care at the right time. Care coordination is accomplished through an integrated team approach that always starts with you and your Primary Care Physician. Depending on your particular situation and needs, our team may also include a Specialist that you may have been referred to, and/or the EHP's own Helping Hands Nurse Care Coordinators.
